shoulder&neck

I went golfing last nov.and next day shoulder hurt me for few days then went away off and on for few months with pain  but past few months is really bothering me,Its been going numb in my shoulder and traveling up my neck and cant sleep on my left side!Oh im 45 yrs old male.

While it is possible that you sustained a muscle strain due to your golfing, it is likely that you may have some degenerative disc disease in your cervical spine.  DDD is a normal process of the wear and tear on our spine as we age and increases with additional stressors such as repetitive movements.
The symptoms you describe could be related to abnormalities in your cervical spine.
It would be beneficial to make an appt with your primary care physician for a medical evaluation and baseline cervical spine x-rays.  Should there be any indication of disc abnormalities it might be necessary for an MRI to see if there is any nerve involvement which would create the numbness and decreased range of motion you are experiencing.
Many times relief can be achieved with anti-inflammatory medications and physical therapy.
