tight neck chest and shoulders

I have had an mri of cervical spine and neck and been diagnosed with frozen shoulder and crps. Mri showed significant scar tissue on the right shoulder and a little bit on the left, bulging disc in neck, degenerative arthritis between collarbone and shoulderblade and bursitis. I feel like I have an adhesive condition all the way across my collarbone in front from one shoulder all the way across to the other, whats going on it has been 20 months and I have already been through extensive physical therapy and nothing is helping. I can't even stand up straight. What do i do and where do I go.
