I know sometimes I have trouble concentrating or focusing on things, especially during mood swings and medication doesn't fully treat that for some people. Also cognitive confusion is a side effect that can occur from some medications. So it would be worthwhile to see when it occurs and if the level of it changes at any time and discuss this with your psychiatrist so they can see how to best treat it. Also I have found there are ways to adjust to some aspects of it such as keeping lists of things I need to follow up on and using a calendar for others that are important such as when medications have to be refilled and when doctor's appointments are.
