I have a loved one who had an old infarct found after experiencing symptoms of a TIA or mini-stroke. The old part for my loved one meant it happened months previously. They didn't see anything from the TIA he had just experienced. I think God used the flaxseed oil capsules he was taking, which have a mild anti-coagulant to the platelets property to prevent something potentially worse happening when he had the "old" infarct happen, which we didn't realize about. He is now taking plavix daily.
I hope the doctor put your grandma on an aspirin or plavix type regime to try to prevent a stroke in the future and is also taking steps to get her blood pressure under control and her cholesterol levels to optimum levels. Also, you might ask the doctor if it is alright to give your grandma a sublingual vitamin B 12 dose on a regular basis to see if it doesn't help her memory. Sublingual (under the tongue) administration of this vitamin can help absorption in older individuals.
Neuro atrophic changes- interesting neuro was mispelled in the report- neuro has to do with nerves and atrophic has to do with wasting away of cells or tissues.
