MRI diagnosis

I was advised to pose my question in this community.
I have several questions that I'm not sure tie together.  About 6 months ago my knee was in great pain from a fall - in the middle of the night I limped to get some pain meds and passed out, hitting my shoulder and left side of my head.  Since then I've had some dull headaches (daily), which I never had before the fall.  I've also had left ear problems for several years and with balance.  I had an MRI of head without contrast a week ago and it was mostly normal, except it stated: "Focus of left subcortical gliosis likely of microangiopathic origin" and "Sphenoid sinus mucosal retention cyst, left."  Would that cause my dull headaches and dull pain in my left temple?
I also have lupus and take hydrochloroquine.  I've read that that medication can cause heairng loss, which I also have in my left ear.  Do you have any suggestions about what i should do or if I should see another doctor, or if it's nothing to worry about?

I would go see a neurologist pronto. I am not sure about the first finding but the second is a cyst in a sinus way back in your head, behind your eyes, causing a clog and that would not feel so great.
Have you seen an ENT about the hearing loss just in case it is not the medication - or if anything can be done to help it? The ENT may help with the sphenoid too.
