I have a rather lengthy question regarding my results thus far.
Here are two separate MRI results note one from the right and the second states that the problem is on the left.
suggestion of rounded T2 hyperintense lesion within the right lateral aspect of the sella measuring 3mm, abutting the right cavernous sinus. there is also suggestion of minimal focus of associated hypoenhancement in the same region on post contrast images. Considerations include a tiny pituitary cyst or pituitary microadenoma. There is certainly no large mass causing compression to the gland itself . There is no mass effect on the suprasellar cistern or optic chiasm.
The pituitary is flattened along the sella floor. The infundilum is grossly midline. On dynamic sequences there is an elongated tubular structure along the left aspect of the sella measuring 3mm By 3 mm. Given its atypical tubular appearance and adjacent to the left cavernous sinus, findings may reflect prominent intracavernous venous vessel. Optic chiasm demonstrates normal horizontal orientation and appearance. Otherwise bilateral cavernous sinus are intact. Bilateral carotid flow-voids are preserved. The orbits are unremarkable. Minimal sphenoid mucosal thickening.
Elongated tubular structure along left aspect of the sella. Finding may reflect either left cavernous venous vessel or atypical pituitary microadenoma. Otherwise, pituitary is mildly flattened along sella floor. Likely compatible with partially empty sella.
Mild prominence of ventricles and sulci in a pattern of mild volume loss.
I have also been told I have osteopenia and polycythemia.
I have also been diagnosed with a mucosele in left sphenoid sinus abutting the dehisident optic canal.(unsure about spelling). This affects my vision. Is this common?
My hormones have been low and low normal for the past.
Here are my most recent.
Three 24 hr urine . high range 4-50 79, 135, 92. I have had 2 normal 24 hr test
three 11 pm saliva test high range .3-4.3. mine were 7.9 and 8.2
one 11 pm >.09 mine was .17
only 1 high plasma cortisol. Many others high normal
testosterone has been low for about 18 months. my latest free testosterone was 44 range 47-244 and 1.8 with a limit of 1.8 to 2.7.
My latest LH was 1 range 2-8.75. Prolactin always below normal
TSH always hovers right on low normal or on the lower limit.
I am interested in your opinions about the right side affected in june then now problem on the left.
Can empty sella cause hormone problems. And I know the hormones can cause my many symptoms.
Can an enlarged blood vessel cause pituitary disorder? Can Thick blood have anything to do with this.
In your opinion is polycythemia and pituitary problems related? If there is anything else you would like to ask me about this condition I will respond, I need help.
I am only a layman - but with your reports, I would go find a pituitary center. The odds of you having a pituitary tumor are pretty high - and even with partial empty sella, you should be evaluated. Empty sella can cause hormone issues - your pituitary can be squished.
The low hormones should be looked at. Looks like you are hypopit or panhypopit - or have Cushing's with the high cortisol.
I have not heard of that issue with the optic nerve - but I am not a doc - have you seen a neuro-optho?
I don't know of a relationship of the polycythemia and pit - but I know with my Cushing's it messed with my platelet function.
