benign phyllodes tumor

benign phyllodes tumor

I had excisional biopsy of my left mass 2 weak back. I got my pathology report today. I never had any core biospy before surgery. We opted to go for surgery , so that we can get rid once for all.
But now seeing my report doctor is saying that they have to do one more surgery to remove some remaining tissues so that it will not reoccur. We didn't know before first surgery that there will be any chances of another surgery.

The pathology report is as follows:
Fragmented breast tissue with fibroepithelial proliferation.
Fibrocystic changes including fibrosis, cyst formation & usal hyperplasia.

These slides from excisional biospy of left breast mass show  frgments of a bengin fibroepithelial lesion with mildly cellular & focal pseudoagniomatous stromal hyper plasia like stroma. Occasional stromal mitoses are identified(average 0-1 per 10 high power microscopic fields). Although a fibroadenoma is a consideration , I believe this is best classified as benign phyllodes tumor. The specimen is recevied in multiple tissue framents & it is uncertain whether the tumor has been completlety excised. Complete excison would be prudent to reduce the risk of local recurrence.

Iam 26 married.I don't have kids. We are planing for baby by end of this year.
So my questions are.
1. Should I go for second surgery
2. What are chances of reccurance with second surgery & without.
3. If second surgery is done, will it effect my health in future in any way. Like problems when I conceive     & breast feeding.
I would appreciate if you can help me with my doubts.

Dear pooja1165:  Phyllodes tumors can be benign or malignant.  Benign phyllodes tumors do not metastasize but can be fast growing, aggressive and recur locally, hence wide excision is recommended.  Malignant phyllodes tumors have the capability of metastasizing.  Distinguishing benign phyllodes from fiboradenomas and from more malignant tumors can be difficult.  Reviewing the pathology with a second expert (someone well experienced with this diagnosis) may be helpful in validating the result and helping with treatment decisions.  In terms of the results of a second surgery, it will depend upon how much tissue needs to be removed.  If the amount of tissue to breast ratio is small, there will likely be no further problems.  If a large amount of tissue is removed, there could be issues cosmetically and functionally.  You should discuss this with the breast surgeon who can discuss this within the context of your situation.

I was in this situation 10 years ago and did not have it redone.  I ended up back 2 years later having a lumpectomy. You absolutely need a clear margin.  It did leave me rather uneven but I had an implant done a few years later after done having children and a reduction on the other side. (better than before!)  You can breast feed on one side if that is an issue.
