Areolar discoloration - lactating mom

I just noticed a discoloration of my left nipple this a.m. - white, ~3 mm, top, center of nipple.  Quite Painful.  Currently breastfeeding twins (6 mo old, ~33 lbs together).Previously breastfed now 4 yrold for 2.5 yrs.  Also had a blocked duct last weekend, but the kids seem to have cleared that although the breast tissue remains tender.  History of mastitis several months ago.  No AB's w blocked duct, just ibuprofen. OB's nurse pretty much blew me off this morning when I called.It is quite painful.Ideas appreciated.  Thanks.

When I was breastfeeding, my breast turned white when my baby got thrush.  It did cause my nipple to be painful.  Both the baby and I had to be treated.  You might look in the babies' mouth to see if there are any white spots in their mouth.  If so, that's your answer.
