Benefit of Tamoxifen in case of ER Negative, PR Positive

by Afroza, Nov 30, 2002 12:00AM
I had my BCS( Rt. at 2.0 Clock Position), 22nd Mar,02. My tumor was with IDC Grade-II , max size of 2.0 cm had ER Negative and PR Positive to Score-6. My treating Oncologist prescribed adjuvant therapy. I started (5th April,02) and completed my  6- cycles of Chemotherapy of CEF Regimen, sandwitched Radiotherapy & Brachytherapy, and on Tamoxifen from April,02. Among so many opinions including MD Anderson & yourselves, etc. that the benefit of Tamoxifen in case of ER negative had little benefit, Prof. Michael Baum, of The Portland Hospital, London opines, that you can't synthesise the progestrone receptor protein without having an intact oestrogen response pathway, as such tumor having rare phenotype, ER negative & PR Positive would also be Hormone Responsive.

Therefore, I am continuing Tamoxifen. Kindly comment.

Dear Afroza, I'm not sure what you wanted us to comment about, so I've copied your question and our answer to your prior question regarding the ER negative/PR positive status of your tumor and hormone therapy.

My excised tumor from my right breast had been examined to be ER: Negetive & PR: Positive (Score-6). Which Hormone Therapeutic drug would be most effective in my case ? I had lumpectomy done in the last week of March, 2002. Please advise.

Dear Afroza: In general, an estrogen receptor negative, progesterone receptor positive tumor will respond to tamoxifen. This would be appropriate regardless of whether your are pre or post menopausal. Aromatase inhibitors, which can be used in estrogen receptor positive post menopausal women, have not yet been well researched for your situation.

Your case sounds very familiar to mine - 2cm tumor, stage II, ER negative, PR positive, HER2-neu 0, aggressive characteristics (high nuclear grade, etc.) After a lumpectomy, I had 4 cycles of A/C chemo followed by 7 weeks of radiation.  My surgeon, gyn, medical and radiation oncologists all are in agreement that Tamoxifen may be beneficial.  In fact, my gyn feels that the PR indicator is the most important of the two anyway.
