Breast change

About a year ago, I started developing small rashes around my neck (scaly patches of skin).  My doctor gave me a cream that prevents the rash and cures it when it appears.  However, for the past month I have noticed that my right breast has had a similar rash.  The skin around my nipple is scaly and there is a bright red circle that goes all the way around it.  When I use the cream, it goes away, but then it comes right back as soon as I stop using it.  It doesn't itch or hurt, but it is flaky and looks completely different than my left breast.  I am very worried about it because I am only 18 and the thought of breast cancer or problems frightens me terribly.  Does this sound like anything serious?  What should I do?
