Cause of Death of my Mother!!

My mother died last month she was 54.
She was diagnosed breast cancer in initial stage it was in her left breast, so the whole breast was removed successfully.
Then she was treated with chemotherapy, radiotherapy and bone scanning (everything went fine) and along with it she was given Femara as a regular medicine with Polybian multivitamin.after 2 and half yrs another doc diagnosed reoccurence of cancer then we reports to mother’s oncologist he gave another chemo dose and said no problem only some cancer cells are in lever and 20%in one lung So after chemo dose my mother had tough week due to vomiting and nausea but it also got fine. after ten days she had breathing problem we took her again to oncologist he checked thoroughly and said my mother had a slight phenomena attack and prescribed medicine and admitted her.In hospital after nebulization she got fine then at night accoding to prescription fo onclologist she was injected "Serenace" and Phenergen" for rest.After Just after it my mother had jolts and she went into shock with pain codnt breathe, doctor gave her Ammonia to counter act the reaction of medicine but her blood became think and was not accepting it, and she was constantly moving with pain.Doctors on duty said the cancer has spread we cant do any thing.
Please help me out I want to know how did she die?  
Was it a mistake of a doctor?
Was it due to injection Serenace?

I agree but people dont die in one day if they have all the medical reports of the person a week ago and oncologist says ther is no problem with the disease. I searched on net Serenace should not be given to the Breast Cancer Patient and Pheneregen should not be given to patient with any of lung problem. And both of these injection were given to my mother when she died suddely with jolts and extreme breathing problems. I want to confirm this one of an other patient died in the samw way my mother died under the custody of same doctor that makes me think it was might be becasue of doctr. My mother has died but I want to save the lives of so many other patients here. In Pakistan ther is no check and law agaisnt doctors so they are free to do anyhting and they dont even bother if someone dies because they think it was his/her time!! My mothers cancer spread due to carelessness of docotr he never made any checkups even my mother complained of pain and cough. We went ot another dodctor he detected the re occurence of cancer, but that was in 20% of one lung and few metastasis in liver.My mother's doctor gave Chemo dose and said it will be fine after a few days, and then when he prescribed Two Injection Serenace and Pherenrgen for Pneumonia that took my mother away for ever,she was only 54.Please help me to find out the reason of her death. She was given Chemo dose just a week ago then why did the doctor give her so strong medicine didnt he know that after Chemo patient is so vulnerable and cant bear any stron thing!!

The doctor here will only respond to one question. And I doubt you will get any doctor to go against your mothers doctor. Unfotunately no matter how wrong they are it is very hard to prove. Try posting a new question here. So sorry about your mom.
