Ductal carcinoma in situ (DCIS)

My diagnosis after stereotactic biopsy is ductal carcinoma in situ. I am scheduled for a needle guided lumpectomy of my right breast and a sentinel node biopsy. The recommendation of a node biopsy was made by the surgeon I went to for a second opinion. His radiologist in review of the pathology slides noted, quote,   " a small focus of cancer that I am not certain if it represents in situ cancer in a lobule or microinvasion." My #1 breast surgeon's nurse reviewed those results and  she told me that the radiologists from both surgeons conferred and agreed that a sentinel node biopsy should be done. In addition, a third radiologist from an affiliate  hospital of surgeon #1 will review the report.
Which terms in the report suggest the need to do a sentinel node biopsy? Does the term lobule mean that there there is cancer in the lobe even though the biopsy was of my breast? Does microinvasion mean the cancer is more aggressive? Other terms "nuclear grade lll, necrosis" makes me wonder about how invasive this is.

All of my tumor fit in the biopsy needle when they found my cancer.  The margins were clear. Yet, the docs removed 14 lymph nodes.  Although it hurt for several weeks, I'm SO glad I had it done.  I couldn't fathom living with the uncertainty of whether it had spread or not.
Good luck with your decision.
