Margins after mastectomy

Margins after mastectomy

I recently had a mastectomy after an unsuccessful lumpectomy.  In the pathology report, under diagnosis, there is the statement, "No residual tumor identified."  However, I am concerned about a few statements in the gross description section and how to interpret them.  It reads like any typical gross description with measurements and, well, descriptions, noting the biopsy cavity from the earlier surgery.  My question is about margins and how they come into play with a mastectomy.  I will quote a few statements from the gross description that worry me in terms of whether or not there is still cancer left behind. "The resection margins are inked green.  The specimen is serially sectioned from medial to lateral in 14 levels.  Spanning the upper and lower outer quadrants, but mostly within the lower quadrant, is a 4.2 x 3.9 x 2.0 cm previous biopsy cavity. This cavity underlies the skeletal muscle and grossly borders the inked deep resection margin.  The inner wall lining of the cavity is markedly roughened and hemorrhagic. . . The remaining cut surfaces are predominantly yellow-tan and fatty, except for approximately 10% white fibrotic tissue which is immediately medial to the biopsy cavity."  Is there reason to be concerned about these areas?  What do you do when a mastectomy doesn't result in clean margins?  Thank you.

Dear wherria:  There is no indication from this report that there are any positive margins.  You may wish to have a coversation with your doctor who can explain all of this in detail to you.
