Persistent seroma post mastectomy

Persistent seroma post mastectomy

I am 9 weeks post right mastectomy and latissimus dorsi flap reconstruction.  After 3 weeks with a Jackson-Pratt drain it was removed and 3 days later I had a seroma in my right flank area that was drained of 160cc of fluid.  A compression dressing was applied and 3 days later the fluid had collected again and was drained of 60cc.  At that time, a 7 Fr Dawson-Mueller catheter was inserted and connected to a J-P bulb to drain.  I continued to have drainage which ranged from 40-20 cc per day.  I have since had sclereotherapy twice where pure alcohol and betadine (equal part solution) were instilled into the catheter which has been in place now for 5 weeks.  I spent 3-4 hours turning from side to side, prone to supine every 15 minutes with the hope of closing the seroma's space to facilitate scarring.  WHile doing this the drain was capped and then let back to drain after the 3-4 hours of medication dwelling in the seroma.  Today I am 6 days post treatment and I continue to drain 15-20 cc per day.  Are there any suggestions for subsequent treatment.  How long can this catheter remain in place and when would it be feasible to remove it?

I had a second drain placement after my 2 drains were removed. I was told that as long as a catheter was left in place that the surfaces would not "stick" together and the drainage would continue. At times a seroma is just left to re-absorb on it's own which does take a considerable amount of time. No professional opinion here; only my own personal experience.     Regards ...
