Radiation in tissue expander

Radiation in tissue expander

I had modified radical mastectomy in lb July 2005 with 7.5cm tumor and 3 positive lymph nodes. Tissue expander was installed at surgery. I have finished 8 rounds of a/c/t and am about to start radiation therapy. Just recently found out there could be problems with the expander and radiation. According to rad. onc. fibrosis and capsules could form around the expander causing pain and future problems. Also, with the metal valve I'm concerned of radiation beam being rebounded into other areas of my body.

Dear crk1000:  Radiation treatment is commonly recommended following mastectomy for larger tumors (usually over about 5 cm) and those with extensive lymph node involvement. Radiation can result in permanent effects on the tissue within its field.  In your situation, our best recommendation is to discuss this with your plastic surgeon, your breast surgeon, and the radiation oncologist to determine your radiation needs, and your best options for a good cosmetic result.  These experts who know your situation, will be best able to make specific recommendations.

Just wanted to let you know I had radiation after my tissue expander was fully expanded (2003) and have had no problems.

I think your radonc is being honest with you.  There are papers published that say the rate of problems with a prosthesis can be as high as 50% after radiation.  As long as you know this going and and are willing to take the risk I would not have the implant removed.  If you develop a problem it can be dealt with in the future.  The radiation should not effect your heart valve.  It should not be in the field.  The radiation dose is deposited in the tissue treated.  There is a very small amount of "scatter" radiation that is produced from inside the body but it is not a problem clinically.
