Sore on Nipple

I have a small open sore on the lower part of my nipple.  I have had it for some time - it seems to get better and then after a month (?) reappear.  I also have occasional yellow discharged from this nipple (mostly when forced).  Are these signs of Paget disease or could they be signs of a not so serious problem?
