I'm not quite sure what you mean by "grade 4.".
"Grade" refers to the aggressiveness of cancer cells, normally on a continuum from Grade 1 to Grade 3.
Could you possibly mean a BI-RADS Category 4 radiology report from a mammogram? That would reflect a degree of suspicion requring a biopsy.
If this is not what you meant, please provide more information, or copy the exact wording from a report you have at hand, and perhaps someone can help you.
