clusters of microcalcifications

I had a mammogram that showed 2 clusters of microcalcifications and a mass.  BIRAD 4A, low suspicion.  recommended excisional biopsy due to thin breast tissue.  I went to another radiologist who took additional films and told me I had DCIS, based only on viewing the films and ultrasound.  It will take 3 weeks to get a consult with a surgeon.  I am extremely worried.  I am 43, no family history, 2 children, no alcohol consumption, low body weight and physically active.  Can a radiologist give a diagnosis based on diagnostic imaging alone?  Should I try to find another surgeon who can see me sooner?

Hi,

It is not possible to give the diagnosis of DCIS on the basis of radiological findings alone.
Did your radiologist give you a copy of the report? Has he / she reported it as DCIS in the final report?

DCIS can only be diagnosed after a needle aspiration or biopsy. BIRADS category 4 - meas the lesion is suspicious for being malignant - a needle aspiration or biopsy is usually done in such cases for a confirmed diagnosis.

You could ask your physician to try to get an earlier appointment with the surgeon. If it is not possible you could try to get an appointment with another surgeon.

This is not beacuse there is an urgency or emergency - but just that the delay in a confirmed diagnosis will only cause anxiety and make you worry.

The further management plan can only be decided after a confirmed diagnosis is available.

Thank you for the information.  I have not seen the final report from the radiologist who told me I had DCIS.  She recommended I consult a breast surgeon and, because there is only one at this particular facility, will take several weeks to get an appointment.  Would a general surgeon be appropriate for my case?  If this is truly DCIS would an excisional biopsy remove all of the malignant areas eliminating the need for additional surgery?  I would really like to expedite this as the worry is taking a toll on my health.
