dilated ducts

I am 50 years old. I had a breast sonagram done of the right breast which indicated the following: High resolution images are obtained and compared to previous study. the previously noted large cysts in the breast are still present. The cysts at 1 oclock in aggregate are unchanged. The cyst at 3 oclock is only slightly larger. There are cysts behind the nipple. There are dilated ducts behind the nipple.
The breast is highly deformed and painful. The surgeon recommended aspiration of the cyst at 3 oclock. This was done recently, fluid did not indicate any abnormalcies.
The breast is still painful and disfigured, it feels like something the size of a baseball (and hard) on the inside. Now they want to do a biopsy. The surgeon said that possibly the cyst developed scar tissue around the cyst and that is what is in there causing the problem. What will a biopsy tell us? He said if the biopsy is inconclusive then I can live with the dilated ducts (I don't see how, too painful) and deformery or have everything within the breast removed.  
It seems from my investigation that no one is really sure what causes dilated ducts and what to do about it except live with or remove them, which in my case is more than what would be left.
I have already had one surgery (aspiration) and now going to have a biopsy and then maybe another surgery. Isn't there something more definitive?

Dilated ducts, called ductal ectasia, can result from the ducts filling with fluid and often this problem will subside on it's own. It can also be cause by an infectious process and this can be treated with antibiotic therapy. If the condition remains removal of the ducts is the solution. This condition can be quite painful. I would advise you to proceed with the biopsy that has been recommended and the ductal removal if everything else fails. Neither one is a terribly "big" surgery and I wouldn't consider the aspiration as an actual surgery.  Regards .....
