Hi. This forum has been very helpful over the past two months. I was recalled for mag mammo in late Oct for a tiny cluster of four microcalcs, Bi-rad 4. I am 53. Referred to somewhat creepy gen surgeon who said he thought it probably would be benign, if not, DCIS. He wanted to excise a golfball piece of tissue as he thought it was too close to chest wall, then radiation if DCIS. I discussed this my primary. She thought she had referred me for stereotactic. She contacted a radiologist and set up stereotactic. Procedure took 2 hours and I was told to expect a hematoma. All 4 calcs removed, although one was expected to be benign. My path included atypia so I have now met with a new surgeon. She intends to biopsy the site in two months if I am healed enough, but says she may see other tissue she wants to excise because there's some dense tissue nearby(always been there) and I will have extensive scar tissue from hematoma. She says that she will not wake me up with a mastectomy, if necessary we will do that after we've had a chance to talk. I'm not flat chested, C cup, so I am wondering how much tissue needs to be taken for biopsy. The tennis ball sized hematoma has moved my titanium chip so she will use my mammograms to locate the area. Path: multifocal ductal epithelial hyperplasia with atypia, duct ectasia, apocrine gland metaplasia, adenosis multifocal, multifocal microcalcifications identified.

Dear sally944,   An excisional biopsy attempts to remove the least amount of tissue needed to hopefully remove all of the area of concern, as well as a margin of normal tissue surrounding any abnormal tissue.  Based on what is seen on mammogram or other testing, reports from the prior biopsy as well as physical examination helps to determine about how much tissue might be taken.  It is impossible over the internet without having evaluated all of the above to say how much tissue is going to be taken in the excisional biopsy.
