my grandmother was recently diagnosed with invasive ductal carcinoma high grade (grade III, Nuclear grade 8).  What exactly does this mean? Her microscopic description ws very confusing as it states Rare ductal/glandular configuration is identifed.  what does that mean?  its says her indivudual tumor cells are rather large with pleomorphic and hyporchromatic nuclei???

Dear kisersassy,  The pathologist describes on the pathology report different aspects of a tissue specimen from a biopsy.  This pathology report gives important information to the physician, which is used for recommendation for further treatment.  

Invasive ductal carcinoma refers to cancer cells that have invaded into the duct of the breast.   Histologic grade also describes the cells in comparison to normal breast cells. Grading system is a way of classifying tumors by their appearance under the microscope.  A "high-grade" tumor has more immature, poorly differentiated (looks less like the normal cell that it originated from), more aggressive looking cells.  A "low-grade" tumor which is usually mature, well-differentiated (looks similar to the normal tissue that it originated from), slower growing and less aggressive. Nuclear grade describes the nucleus of the cells in the tumor.  The higher the number, the less the nucleus looks like a normal breast tissue cell.

The other information on this pathology report further describes the tumor cells.  Pleomorphic meaning: the cells seen are in different stages of the cell cycle.  And hypochromatic means the cells have less color to them than normally would be seen.

The surgeon will be able to put this information from the pathology report into context for you and your grandmother.  The information from this report as well as other information (mammogram reports, physical examination, overall health of your grandmother etc.), will be used to determine the next step and what would be the recommended course of treatment for your grandmother.
