This soreness can have several causes. First, if you are still pre-memopausal, hormonal changes can induce changes in the breast which can give rise to pain and swelling. These are most evident just before one's menses and usually subsides by the time you menstruate. Another cause is like what you have in mind, musculoskeletal problems. This could be problems with under or overstretched muscles and soft tissues in that area. Also, the incision may have damaged some sensory nerves in that area giving rise to pain even after years from surgery. I agree with your surgeon consult for a better evaluation and examination.
