second opinion?

Had small amt. brownish clear discharge fr rght nipple for the 1st time.  Called dr. and they got me in within 24 hours.  Had mammogram & ultrasound.  Told dr. had swollen lymph nodes(glands?) @ groin area for about a month.  Have since gone down in size.  Were about the size of an almond. Forgot to mention 2 in neck directly under chin that I can also feel, size of a very small pea.  Noticed those about a month ago also.  I have noticed a difference in the shape of my right breast.  Not a lump exactly, but more like a small firm mass (my breasts are small) above nipple.  Didn't know grandma had breast cancer as result of colon cancer (mother told me after appt.) so I didn't mention it.  She had cancer throughout her body when she died.  1st dr. noticed on visual exam right breast looked different, "bulged" out.  Mammogram showed too much density? so they did ultrasound.  Ultra sound apparently showed nothing.  Have had both boyfriend and brother in law comment on how my breasts have been growing over the last couple of months.  Could they simply be changing since I'm getting older? Bras do seem to be getting smaller.  I'm 38 yrs. old.  About 10 years ago had lump in left breast aspirated.  It was nothing.  I smoke heavily and have smoked since age 16.  Have been on birth control approx. 20 years.  

Doctor called after he reviewed preliminary report.  He is NOT the doctor who saw me and said that it was nothing to worry about, just a "thickness in breast tissue"  Is there a need for a second opinion (other than simply to ease my mind?)

Dear JMP, Breasts are made up of ducts, lobes and fat. When a mammogram is done it shows a two-dimensional view of a three-dimensional structure like an x-ray.  Breast tissue is very dense and shows up white on a mammogram.  Fat, which is not very dense, shows up as gray.  A younger person

Forgot to mention that doctor performing exam told me that it was "granular??" under my right nipple.  What does that mean?
