what to do

I have a hard lump under my right nipple. My breast is swollen and and has thickness.  I also had  a swollen lymph node in my neck for a month or so. Breast cancer is in my family. How soon should I see a doctor? because I have no insurance. Also i run fever and feel nauseated.

Since you state that you have a family history of breast cancer,it's very important that you seek medical intervention the sooner the better.Lumps anywhere in the body should never be ignored.It could be that you have an infection or a benign tumor, but it's better to make sure and have peace of mind.
If you have no health insurance you can contact your local department of health and human services or department of social work (should be in the phone book). In most major cities, there are medical facilities that will take fees on a sliding scale based on income or have plans for people who do not have adequate insurance coverage.These offices should know where you can obtain medical care in your area.  
You also might try to contact The Susan G. Komen Foundation.There should be an affiliate in your area.
