Are they the same? What do they mean?

Results of XRAY from last year is "ill defined opacities in both upper lobes with homogenous density in the right lower hemithorax obliteratin the right hemidiaphragm and sulcus? hearts is not enlarged, other visualized structures are unremarkable. Impression; PTB, Consider pleural effusion, right"

This year xray result is "Fibronodular infiltrates are seen on both upper lobes, heart is not enlarged, diaphragm and sulcus is intact. thoracic spine is slightly deviated to the left impression; ptb, mild dextroscoliosis"

Did it became worse or kind of healing?  Please tell me. THanks
