Hi,
How are you? The rate error varies depending on the diagnosis, the procedure done, the symptoms present, the institution, and the pathologists who reads the biopsy slides. Although it is very difficult to say the exact rate, one article suggests an error of 1.4 percent. It is highly recommended that a second opinion is sought before undergoing any major surgery, chemotherapy or radiation. This is also true in negative biopsy on an organ that is known to have a higher rate of error  ( ref: http://www.nytimes.com/2000/04/04/health/value-of-second-opinions-is-underscored-in-study-of-biopsies.html?pagewanted=all&src=pm)
Take care and regards.
