lump in armpit lump,pain,nipple pain

I had armpit pain and lump (appr.1" long 1/2" diameter) appr. a year ago. I had a mammogram and it showed up but the ultrasound did not, so the Dr. dismissed it. It has grown considerably (appr.4" long and 1" diameter) and painful.There is now soreness in my nipple. My Dr. does not seem concerned but I am since this has been ongoing for over a year now and spreading. I am considering making an appointment with a specialist but the cost is a hardship and I do not want to go and find out that it is nothing to worry about. Does anyone have an idea of what it might be. Much thanks
