swollen lumps on left testice and left side of neck and behind left ear and jaw

I have had a swollen lump that would get irritated from time to time and swell even more with almost what feels like a web of swollen vains when irritated as well for about 5 to 7 years now, and since then have also devloped a lump on my neck which makes my neck sore as well as my upper back when it swells. I havent been to the doctor about it though. Now  in the last week ive developed several swollen spots (glands/lymph nodes???) behind my left ear and hehind my jaw under my chin and down the left side of my neck. Its painful constantly and im dizzy constantly sometimes have spells where it gets worse and i can really think clearly , i also almost passed out the other day for about 15 minutes my vision went blurry and grey it was almost like a dream or hallucination, i got really warm  and it was kinda hard to breath. There is a cluster of swollen lumps clustered around eachother in my neck all about 0.5 to 1.0 cm in size, the one behind my ear is about 3 cm in size (what can be felt of it that is, its petruding from behind the left ear almost pointy in shape) there are 2 next to that about 1.5 cm in size that lay on my skull behind my ear, and one hehind my jaw thats almost an inch in diameter it causes a large visible lump when i look to the right. these are all on the left side and its getting bad enough i dont like to turn my head because of the discomfort.  What im wondering is what could this be , ive been reading about similar symptoms and it seems these are often associated with lymphoma and cancer, I dont have a family doctor so what should i do , go to the hospital and request a biopsy. Im getting very worried about it because my testicle also flares up when the glands/nodes in my nech etc get irritated. Im 25 i smoke and i also have a few cysted eating at the bone in my righ arm, could this be related to cysts im really at a loss as to what is wrong with me and with no doctor, id like to have somthing to go on when i go to the hospital.
