This is definitely a medical issue. She needs to go to the vet ASAP. I would also call the shelter and ask them if they knew about this behavior. That will give you an idea of how long this has been going on.

I think you should see the vet. This is not normal even for outdoor cats. They are very private creatures with regards to their "bathroom" habits. I think perhaps she has a UTI--or urinary infection, or some other bladder issues. I am assuming each time she has wet in the house you have put her in a litter box.
Perhaps she does not like the litter you are using, or where you have her litter box. It should be in a quiet place with privacy, almost hidden away from everyday activities, like the laundry room or bathroom.
I am going to guess you have done all this, and I think I would investigate some medical issue rather than a behavioral one. Seems like she has the good life with you.
