resources for a CA geriatric CP patient- desperately needed

My relatives in CA, the Los Angles County area, have a 74 year old relative living with them. She has had CP all her life, with recent deterioration in the past few years. Short term rest home placements and even in home care are becoming nearly impossible, as these relatives are retirement age themselves andf she is almost total care, with some mental deterioration and disorientation, loss of cognitive skills. She is currently in hospital for a severe infection. This is a change in the past few years as she had worked and was married for many years. My relatives are exhausted with her care. I live in the East.

Are there any geriatric neurologists in the Los Angeles county area whom they could consult? Or specialists in CP in the elderly? Also, are there nursing homes who can manage CP? What if it turns  out to be dementia-alzeimers on top of the CP?

Are there geriatric case managers, public or private who could help them search for or get her into an appropriate nursing home if needed?  There is no coordination of care right now. My fear is that her current hospital will just discharge her home or send her to some general purpose facility where they just don't "get" CP and it would be inadequate care. Finances are also an issue. How would she afford nursing home care?

Any provider lists, specialist lists or resources you could tell us about, would be greatly appreciated.

You might look into SCAN health plan, which at least used to cater more to the elderly.   They offered more help to patients so they could be better cared for in their own homes- used to offer pre-fixed meals at low cost, diapers I think, rides to the doctor, even used to offer maid service at a low cost.  I don't know how they are now, but you might try calling them.
