Chiari Hernation has increased. Do I do anything?

In 2003, I had a CT for neck pain after a minor car wreck. An MRI was then requested and I was diagnosed with mild Chiari with cerebellar tonsils extending to near the ring of C1. I was referred to a neurologist, who said it was mild, and nothing to worry about. Ok, Fine.  So, now, my doctor order an MRI to just check to be sure the malformation has not increased, etc.  I repeated the Cervical MRI today and now the findings are Chiari 1 malformation with tonsillar herniation greater than 5 mm.  So, my question, other than lifelong headaches (migraines), I began having odd symptoms over the past few years that may or not be related.  3 years ago, I started having double vision (an eye muscle issue was diagnosed and suddently, at 44, my right eye is started to turn out). Also, I have this burning pain up and down my spine and up to the back of my head (like a pressure gauge about to explode).  And, when I cough or sneeze, I feel like my head is going to explode!  I have had migraines since I was 3 (no kidding), but these headaches are different in nature and the pain is different then the usual migraine I used to get.

I am also concerned about the "greater than 5 mm" issue.  Is there a "magic" number that demands attention (besides symptoms of course, but I have been living with mine for years, and compared to other people, mine just don't seem that severe)?  Rest of MRI noted on significant spondylosis (arthitis) and some mild disc protrusions.

I know I should just make an appt with neurologist. But my symptoms are pretty vague, and have been around for awhile.  I guess I am just looking for some insight about greater tahn 5mm, but no syrinx, etc...

Thanks for your time

Hi,
There is an illness or condition witch is sometimes associated  with CM, its called Nystagmus, it is an involuntary eye movement and may have something to do with your eye problems.

There is no magic number.  It really is all about how symptomatic you are.  You really need a neurologist who is experienced with ACM to do a thorough neurological exam and symptom history to assess your next move.

Good luck, I know how confusing it can be.

Hi and welcome to the Chiari Forum.

Chiari is a congenital condition which can cause symptoms for many yrs b4 a fall or MVA triggers the herniation to grow. It can grow very fast or slowly.

Not everyone will have the same symptoms and since we have had this from birth we tend to over look some symptoms as they appear normal to us since we have always had it.

U need to find a chiari specialist to see if u have a CSF blockage.

I do not understand ur first mesaurement of the herniation, but chiari under 5 mm can cause more issues sometimes than the larger ones.....but most drs do not recognize chiari until it is at least 5 mm......

Many with chiari also have bulging disks and stinosis,  and other spine issues.

Let us know where u live....view our chiari specialist thread and research the drs.
