So I located doctors in New York, hoping they would be able to give me more information.  I had to fill out so many forms and then they said they would get in touch with me and this can take up to 4 -6 weeks!  Is this the normal procedure?

  Yes, this is very typical of all NS's office's.......they may even ask you to have more testing if they feel what you supplied were not enough.

Try to relax, take a deep breath and let us know when you hear back.
