26 month old only weighs 22.6lbs

I have a 26 month old son who was born 2 months premature. he weighed 3lbs14oz at birth and did fine in weight gain until around his 1st birthday. since then he has only gained 2lbs. he currently weighs 22.6lbs and is noticeably smaller then other kids his age.he looks like he is only a year old! i give him carnation instant breakfast once a day as directed by his pediatrician and have been for a year. he is a picky eater but he does okay when it is time to eat.im wondering what else i can do besides making sure he has a high calorie high fat diet. i know that being too underweigh can cause health issues and i would like to do whatever i can to help him gain weight so that is not an issue. please let me know what i can do

This is not an area in which I have expertise. You have clearly consulted with your child's pediatrician, and that of course is sensible. Consider posting your inquiry on the Child Nutrition Forum.
