4 year olds problem

Can you please help me. I look after a 4year old boy who seems to have something different about him and I can't work out what it is. His mother has now come to me and asked if I believe there is something not right because of what she has noticed at home. Bryce is a very sensitive young man but sometimes have full on panic attacks out of the blue. When this happens he finds it hard to bring himself back down. Bryce is very loud in volumn and is always reminded to turn down his voice inside. Due to Bryce coming to daycare everyday for all his life he should know the routine but he still asks simple questions and needs constant reminding. His attention span is very very short even on activities inwhich interests him. 5 mins max. If you change his routine he is unable to refocus and becomes very distressed. I am very concerned about his cognitive skills. What do you think?

Some children do not like their routine changed.  This does not mean that there is anything mentally wrong.  It is a personality trait.  My daughter was like that up until the age of 5.  She is very bright and excels in school.  You will need to write more of what he is doing, because what you write does not seem out of the ordinary for a 4 year old.  A lot of 4 year olds have limited attention span.  Most 3 or 4 year olds have a hard time finishing a board game for example.  He needs security and is looking for it in the daily routine that he knows.  He will act out if the routine is broken, but it is not due to mental issues, but more do to insecurity.  He needs a secure, stable environment.
