5 year old Urinating on the floor

My daughter has just begun urinating on the floor.  She has done this 3 times; twice at home and once at school.  Any one have any ideas on how to address this?  Anyone with experiences?
She gives lame excuses, but I want it to stop.  She has been fully potty trained for over 3 years.

That sounds like a good strategy.  Maybe she is just a little jealous because you have spent attention on potty training the baby sister and there is a new baby on the way.  Maybe it is just a phase.  Good luck.

I don't think an infection is the answer, but I will check with the doctor.  
We are due with a new baby in Aug, but she already has a baby sister (3yrs old).  The youger child did just complete potty training and they got bunk beds for Chirstmas.  
My 5 yr old has used excuses as someone was on the pottty and there was no one to ask to go.  But, there are many bathrooms in both settings and someone is always around.
We have just institute her mandatory potty time about every 45 min.  If she doesn't potty, she can sit on the potty for 3 min.  She has gone 3 out of 4 times tonight.

Does she have a possilbe UTI, bladder or yeast infection?  She might have one without knowing and this could cause her to not be able to hold it.  Is there a new baby in the house?  Is she emotionally upset? It isn't normal for a child to just start doing that after three years.  I don't think she would do it on purpose as you haven't mentioned any other behavioral problems.
