5 yr old hitting/kicking teacher at school

My 5 yr old daughter has some anger and self-esteem issues and has been seeing a counselor for play therapy for the past 6-months. She's made great progress but  she still becomes angry in Kindergarten when things don't go her way. She will use abusive language toward other kids (you're not my friend, i hate you) and when the teacher intervenes to re-direct her, she will hit and kick the teacher. She never hits/kicks other kids...only the teacher. What is the appropriate way for a teacher to respond when this happens? I'm looking for some tools on handling this type of behavior. The school is becoming frustrated and I don't want her to be asked to leave.

This requires straightforward discipline. She should be placed in time out immediately for 10-15 minutes. This should occur any time she acts in an aggressive manner.
