7 year old son refuses to go poop in the toilet

My son is 7 years old and just refuses to go poop in the toilet and the only time he does go poop is at night when he is sleeping. He has done this ever since he was potty trained and he urinated fine it is only the poop he has issues with. His doctor does not now what to do either. We have tried just about everything punishing him, taking away fun activities, and even sitting him down and talking about it with no change. Now he has gotten to the point that he hides it and it does not seem to bother him that he is doing it. He will sleep all night in poopy pants. We just do not know what to do with him nothing seems to work. We have tried Miralax and that does not seem to do anything either. Can someone help with this issue.
