7 year old with encopresis

My seven year old was diagnosed with encopresis a year ago- we have done everything we were advised to do- her diet is high in fibre, we work hard to get her 6 glass of fluid in per day, we work hard at the bathroom routine along with using a prescribed laxative. its a year later and it is not any better- in fact she often has pee accidents now which seems to come and go in waves but is getting to be an even more regular occurrance than the bowel movements. I am at a loss for how to help her and am starting to worry about the social implications as I often pick her up from school and know she has had an accident. Why is it not getting better? why is she also having pee accidents? any ideas???????
