Behavior problems with child of DS at age 5 1/2 yrs

My son Alex is 5 1/2 years old with DS. Beautiful child with impulsiveness behavior. He constantly spills drinks on pupose, spits, and pulls hair. He has done this for a really long time. We have him in a special needs kindergarden class and have tried to work on this behavior. He is really doing this with us, his parents but for some reason he does NOT do this with our nanny who watches him after school and when he is out of school for 8 hours at a time. I believe that medicine would help but would like your recommendations please.
