Concerned about 4 y/o behavior

My son is 4 y/o. (He will be 5 in Sept.)  Before the age of 3 he was diagnosed with a speech and language delay, for
which he is currently receiving therapy.  I am concerned that he may have another developmental delay.  He has certain behaviors such as:  he is very obsessive about certain things (trains is a consistent one, but he also gets 'stuck' on others); he is overly sensitive concerning wearing clothing with tags or any embroidery backing, shoes, and socks (there have been instances were I needed to restrain him to get him dressed);  he also is overly sensitive about 'boo boos' (if he feels he has a cut, scrape, etc) he will have a 'nervous break down' until he gets a band aid; there are times when he 'wakes' at night upset about something but is not fully awake and there is no consoling him; when he gets something in his head he cannot let it go (if he wants to do or have something he will constantly ask for it until it escalates to a frenzied begging); he has a hard time getting along with his 2 y/o sister (he will intentionally push, hit, or just be mean to her); he has obsessive tendencies (his current one is when he is upset or anxious he pinches his nipple).  These are just a few examples of the behaviors that are concerning to me.  I would appreciate any advice.

Thank you for your response.  I actually had not even considered the SI diagnosis.  After looking at several websites, I am 100% sure that my son has this issue.  It is nice to be able to put a name to the behavior.

Hi Tracy,
As you have probably figured out he may have sensory integration issues.  I would have him seen by a developmental pediatrician.  Our ped gave us the name of a local dev. ped.  and she has been very helpful with our daughter.
