My 4 year is out of control

My son is 4, will be 5 in March. He is having problems with hitting and beating up on his 2 older sisters, he has even hit me before. He will just randomly throw items across the room or directly at someone if they make him mad or get in his way. He gets in trouble at his preschool for walk/running all over the room, he flips the chairs over in class and argues with other kids because he doesn't get what he wants. When we go out in public, he runs off in the stores and thinks it is funny, he will pull tags off of items and throw them all over the store. When we are home and he is asked/told to clean his room he goes in his room and makes a bigger mess and blames it on his older sister or he hides things and says it is clean. He tears up almost everything he touches. I have taken thing away from him, put him in time out, spanked him and have even had to go as far as holding him down to make him settle down after not getting what he wanted. He doesn't listen to anything I say he thinks everything is just funny. Is this a sign of ADHD or any other kind of problem that can be helped medically? I am desperate at this point for help
