My 5 yr old pooped in a cabinet and said voices told him to. Any advice???

My son is 5. He has never really had any issues he was a bit slower developing than my other kids. Like he didn't crawl or walk as fast. When he first learned to talk he didn't really have any issues when he was 4 he developed a studder to where people couldn't talk to him on the phone bc they couldn't understand him. Anyways that went away. He is in kindergarten now and he always gets good marks in his agenda but he was recommended for a class that teaches social skills. He does have behAviour problems at home. He spits on his sister when he gets mad no matter how many times he has been spanked or had things taken away. He also will not clean his room no matter how much I try I eventually get too frustrated and end up doing it myself and by clean room I mean just pick up his toys anyway today I was extremely disturbed to hear my son whispering in the bathroom and opened the door to find him in the cabinet pooping he immediately jumped out blaming his sister. By the way there was like 5 or 6 piles of poop so this was not the first time. I didn't get on to him because I was more concerned. When he finally admitted it to his dad he told him that the brain inside his head told him he had to and then his hand brain opened the cabinet and then his butt brain just did it. I'm concerned and realise this is not normal behavior. It's just I have never dealt with anything like this before and need some advice!!!

Hey you need to get your kid to see a psychiatrist. First visit your family physician.
