Sweet 4yr old with super bad behavior issues

My granddaughter is very sweet but can go from an angel to a monster in a spilt second.  Her aggression towards her mother and at school is progressively getting worse.  When she has fits she hits, kicks, throws, screams.  She can direct this to anyone or thing that gets in her way.  It is impossible to reason with her on any level. She can be happy one minute and very quiet and pensive the next. Her mother has her with a play therapist which really seems to have accomplished nothing.  We as a family are consistent with the 1,2,3 for Christian parents theory.  Again it was supposed to work wonderfully and is not producing any marked results.  Time outs do get her calmed down.  She has trouble going to sleep.  Mother and father are getting divorced after 8 years of a bad marriage. Father never really involved with daughter and is now less involved.  Family background: History of chronic depression in several people; severe and mild bi-polar disorder in two people; problems getting to sleep for my father, me, and all three of my children; one son has ADHD; grandfather who was 40 years older than grandmother was diagnosed as schizophrenic. They say they can't diagnose her but we are desperate.

Hi grammy.  Sounds like you're going through a rough time.

Is it possible she has fetal alcohol syndrome?   Her facial features are consistent with that.  She's a cutie pie - and kids can have the classic FAS features (flat nose bridge,  wide set eyes,  thin upper lip)  and not have FAS.
