i have a 5 year old who do so bad at school

Well my son is so smart he can talk to you like his a teen boy,but my question to you guys is when he's in school he never do his work always playing in the bathroom playing fight with one of his friend from class,not reading or writing when been told from the teachers.Also when he's home i always have to tell him something more then one time.So i want to know what can i do to help him do better at school with out any medical

The problem lies with the school not your son, it sounds as if they have no control , why is he always in the bathroom ? Speak to the teachers and ask them to stop him  fighting and going into the bathroom unless he has to use it.
