potty regression

My nephew is 5 years old and has been fully potty trained since 3 years old.  He never had any problems with potty training, he was really easy, for bedtime also.  If he had to go to during the night he would get up and go to the bathroom. NOW - my sister-in-law says that he is having accidents in his underwear (urinating & bowel movement).  He does it at home and school.  What could be the cause of this?  He can be a quite little boy at times, enjoys playing with other children, usually a happy child and is the only child.  What could she do? OR What should she do?
