vocal tics

Hello, I have a 5 year old son who is in Kindergarten. It has been brought to my attention (again- once in pre school as well) that he has trouble being quiet durring work times or story time. He "clicks" or clears his throat, makes a sound with his cheek etc. when asked to stop I don't think he can, teachers initally think he is just being defiant- but I have seen with my own eyes that he just can't stop- not really sure he knows he's doing it- so the question is, is the ADHD or tourettes? Or what do I do?He is usually well behaved and can concentrate on homework, a movie, puzzle etc. loves to draw. however if he is playing the sound effects are an absoulute and loud.
He is very bright and very loving- but on other days he is like jeckyl and hyde. Fine one minute and off ther handle the next- overly excited when people show up at the house etc... to the point of being too hyper- (almost like when a puppy comes in and runs around crazy until he can settle... same thing here)
And if he is mad it goes to the extreme over little things and as quickly as it came it subsides...Is any of this related?

Thanks sorry this is so loing just thought the more info the better- (also have tried feingold diet for  a short while no sure of the effects- didn't commit 100%)

TD involves both vocal and motor tics, so that does not sound like what is going on. He is on the young side to evaluate ADHD, simply because development has not yet had a chance to run its course. Your description indicates that he tends to become overstimulated in some situations and then has a hard time settling himself. Is this simply immaturity or indicative of something else? It's hard to say. You might consider going ahead with an evaluation and see what you make of the feedback.

Thanks doc- That's what  we've discussed before- maybe it is just his age and he's a "boys boy" or possibly a sign of things to come- i think I will have it looked into thanks again
