what do I do to help a child to stop smearing feces?

I am presently a foster parent for a seven year boy who repeated smears feces here at my home, his school, or anyplace that he is visiting.   I have had him clean it up and have discussed with him that it is a health hazard for himself and others.  I have asked him to try to stop when the thought comes in his head.  He shows no remorse.  He just stares at me, he does admit that he did it.
this young man is polite, appears happy when with other children, knows all the correct social cues, but in a blink of an eye, he is smearing, or he will hurt a smaller child with a punch or pinch.   When confronted, again, no remorse, just a blank stare.
He does have a diagnosis of language impairment within the Autism Spectrum--high functioning.
He is presently taking Concerta every morning and Respidol in the evenings.
I would just like to know what I can do to help erase the behavior of smearing without further damaging the child.  I want to keep his dignity in tack and help him respectfully.
I thank you for your time in responding.
Nana

It could be he is upset by something and this is his way of expressing it ,did  he had a a hard time with his original parents ,my gut feeling is that he is crying out for love and attention, and he is acting out because he feels that need,You may also want to check out the dosage of the meds with the doctor as I do know some of the meds they give children do have some side effects .does he take these two for the Autism ? Perhaps some positive attention ,does he like sports,? will his foster dad take him out for ball games and fun, has he any  foster siblings, how does he get on with them...It does sound by your post that you care a lot for this young child, and want the best for him , thats good , could be he because of his background needs that bit extra ...good luck

I have a seven year old step son that does that.  A woman going for her doctorate in psychology advised me it may be sexual abuse. it is very abnormal at this age.  i don't know what to do, as i am only the stepmom and the parents just throw him on meds.  he has other problems though.  he stilll wets the bed and has a speech impediment and is very destructive.  talk to his doctor.  they can advise you on what to do
