my baby wont take food or milk

hello
my 22 week old baby wont take milk or solids. She was born at 7lb 11 and is now only 13lb 8. She started off taking milk very well from birth ( formula) as she would not latch onto the breast. After a few weeks she decided she didnt like milk anymore and we had to practically force her and would get maximum of 0 oz down her a day. At nearly 4 months old i started to give her the baby rice which she loved for a few days and now she will no longer take i have tried home made food, all the baby pots and flavours possible with no success. She had  2 weeks recently on the ellas kitchen organic food which is pears peas and brocoli and a few other flavours. She has now decided she no longer likes this. She does love pears but i know my child can not live of pears 24/7. i am so exhausted now with trying and failing with everything. I have trued her on a few different milks inc dr prescribed milk , lactose free and the normal cow and gate. Nothing is right for her.
When feeding her the milk she may take 2oz maximum when she is tired and wakes in the night, i have no chance of giving it her during the day as soon as she ***** the bottle and realises what it is she screams until i take it away
when giving her food she will open for the first 2 spoons and then she will shut her lips and even scream and cry throughout the forced feed. she was born on the 50th centile and is now below the 9th.
i understand that food is not so important at this age and a few spoons will do but as she is not taking the milk i am getting so worried about her,
please help, the doctor has refered us but iv not heard anything back yet
x

Her weight and height are below the 3rd percentile for her age. At 22 months she should be feeding herself. Let her eat finger foods with her fingers and give her fluids in a 'sippy' cup instead of a bottle. Hopefully being independent with feeding will help her with eating more food. Suggest giving her Pediasure or milkshakes (milk and ice cream blended) in place of milk, at all times, for additional calories and protein. The most important thing is to RELAX at meals and not force feed her. If she will not eat a certain food, do not make a fuss over it. Give her some other food alternatives or foods she likes. Then let it go and enjoy the meal. Slowly introduce new foods, and if she does not want it, do not make a fuss over it. Sometimes it takes 5-9 times to introduce a food that they will accept, however, if they do not want it and let it go. Try again in a few months. Hoped this helped you.
