Chlamydia or PID?

I contracted Chlamydia about 5 years ago and had no idea I had it. A few months ago, I started having pretty strong symptoms: very uncomfortable itching and burning in the upper part inside the vagina and unusual discharge. At the time I was living abroad and went to the doctor there. The doctor said I had inflammation of cervix, and my test came showing Chlamydia. The doctor put me on antibiotics. I had to take a medication called Wilpraphen ( active ingredient Josamycine which is a macrolide just like Azithromecin ) for 10 days 500mg two times a day. Right after Wilpraphen I had to take Doxycycline also for 10 days 100mg two times a day. While I was taking Wilpraphen, my symptoms disappeared, as soon as I stopped taking it  symptoms came back again even though I was taking Doxycycline at that time. Recently I came back to US. Ever since I finished that treatment my symptoms got a lot worse. Not only I have discharge now, I have blood in it. Occasionally,  I have unbearable burning in vagina for 3-4 hours. I developed a pain inside on the right side close to hip, and a couple of days ago started having a weird sore throat that comes and goes randomly. The sore throat is not strong enough to bother me, but it is noticeable. It feels really weird because I don't get sore throat easily and it doesn't feel like my regular sore throat, it is deep in the throat. I'm suspecting that I have PID rather than a simple uncomplicated case of Chlamydia. Am I right? I've gone to the doctor already and now waiting for test results. The doctor also told me that if the result comes back positive for Chlamydia, she will prescribe me a single dose of Azithromecin. I'm worried that it won't help because I've already had even stronger treatment and it didn't help. What should I do? What should I tell my doctor to persuade her that I have quite a serious case that requires stronger treatment?

Has she done a pelvic exam?  If she has, and didn't find anything, then you don't have PID.  The sore throat doesn't have anything to do with this - chlamydia is rarely, if ever, transmitted orally.

If you don't have PID, even with the bleeding, azithromycin should clear it up, but I'd wonder why you didn't respond to the doxy.  You don't mention any sexual partners, so I don't know if there is a possibility that you have been re-infected by a partner that hasn't been treated.

What makes you so sure that you got chlamydia 5 years ago, and not sooner?  Have you had no sex in 5 years?  I ask because it would be very unusual to have it that long with no symptoms.

I'm sorry, I didn't express myself correctly. Before my last partner of almost 5 years ( who by the way has no symptoms)  I had sex with a man and had symptoms a few days (maybe a week) after that, but I attributed them to some urine infection, besides they cleared right away and I felt fine since then up until about a years ago when I started having rare mild symptoms and four months ago I went to the doctor the first time. My current partner as I said has no symptoms, no complaints, he was tested, we are waiting for his results now as well as mine. It would be unusual for him to get infected from me and not have any symptoms for almost five years. I want to believe he is not infected. We did not have sex while I was taking antibiotics, although we did have sex with a condom after. As for Doxycycline ineffectiveness, I could think of two reasons: one, the person I contracted the infection from tried to treat it with Doxy before we got together, now my Chlamydia is resistant to Doxy; second, I eat a lot of milk and yogurt on a daily basis and certainly did when I was taking Doxycycline, but lately I read in one source that Doxycycline should never be taken with milk ( milk makes Doxycycline ineffective), while another source I found says you can't take milk  with tetracyclines except Doxycycline and some other antibiotic of the same group. I don't know which source to believe. So my consumption of milk and yogurt may or may not be the reason why doxy didn't help at all.

If you've been having unprotected sex with him for 5 years, chances are he'd be infected.  If he's positive, I'd have to wonder about someone going outside the relationship.

I don't know for sure about the doxy and milk products, but from a quick google, I found that it might decrease absorbency very slightly.  Since you were on it for 10 days, I don't think there would be an issue.  The normal dose for chlamydia is 7 days.

If your partner is negative, I'd wonder if you have bacterial vaginitis or something that is causing a false positive on the chlamydia.
