STD infection - is it gone?

About a month ago, i had sex with the "last train home" after a long night drinking. She performed unprotected oral sex on me and then we had sex with a condom. Looking back now, she seemed to be the more-than-average frequent bar fly... she's probably got a bit of mileage under her belt... The very next day, i seemed to have an itch in my urethra, just towards the tip and a slight burning in the same area during urination. It didn't go away after some time, and I noticed a small reddish lump under the tip of my penis. I decided to go to the hospital (this was 3 weeks after the sex). The doctor listened to my story for about 5 seconds and said "you have chlamydia". I was then treated with 1g Zithromax (Azithromycin). He instructed me to come back if there was any discharge from my penis after 1 week. So far, one week later, no discharge, but i still have reason for concern.
All burning / itching sensations are gone, however:
Lately i have been feeling a dull, but noticeable pain in my testicles, and this has been steady for the last 4 days. Also around the tip, just underneath the exit of the urethra the area is still slightly red, and the small lump seems to be a bit more profound. With this information, i feel like I need further treatment, but what has me worried is the color of my tongue has seemed to change to an orange color. It looks as if i had been eating an orange flavored lollipop for some time. The "bacteria" or whatever it is that grows on your tongue has changed color, not my actual tongue. Along with this, i have a very very slight cotton-mouth-thing(for a lack of a better word) going on. I started to notice this 3 days ago, and i'm not sure what's going on.
Did i clean out the STD infection in the first place? Could i have some sort of other infection or possibly even chlamydia / other in my mouth? Is this a side effect of the azirhromycin? Do I have more than one STD?
My ultimate fear is HIV, though i'm able to partially able to dismiss the thought by further research... I should be tested to confirm this. This last month has really been hell and the stress is starting to take its toll. The main thought going through my head is: I'm 25... what if? what then...?
I plan on heading back to the hospital, but i'm not as confident in their examination. Although it seems obvious to seek other medical advice from another hospital, but right now i'm in a foreign country, and I will be here for an additional 6 months so my choices are extremely limited. This is why i'm asking for your advice.

My focus has moved to the bump on the tip of my penis. I'm really not sure what this could be. It's a small, soft, darker colored bump. Could this just be damaged tissue from the initial infection from chlamydia, or is it something else entirely?
